At the start of a run, Neighbour 14 has a 25% chance to appear, and if it does then it will appear in the first sewer area that the players visits out of:

Strategy

Fear & Hunger Mode

The Neighbour is a formidable foe for small parties. Both of its arms will use Sack Smash every turn, dealing medium damage to any party member hit.

During combat, there is a 50% chance for a Neighbour to "stick out their tongue" each turn. Once they do, it means that the following turn they will use a skill called Suction Tongue. The skill consists of three successive attacks on a random party member, dealing heavy damage and likely killing them.

The Neighbour warns the party before their coin flip attack with, "The floppy arm is tensing up...". It is best to get rid of the arm or guard after this text appears.

It is recommended to destroy the Neighbour's head or torso as quickly as possible, as its arms have higher health and destroying them will not prevent its Suction Tongue ability.
